The Diabetes Program works to identify, provide education and health care services for the diabetic population which are the type 1, type 2, and gestational diabetic; and who utilize the I.H.S. and/or Tribal Health Department for their diabetes cares and management.

Mission, Vision & Goals: 

​The mission of the Cheyenne River Sioux Tribe Diabetes Program is to promote wellness and prevent and treat disease within the community, as a health care team, the Diabetes Program will provide comprehensive services and holistic care the clientele who utilize the services of the program.

             2. Diabetes Education

             3. Case management 

             4. Community based diabetes awareness events including health fairs and walks
